Referrals
The referrals process into the team was altered in order to make this process more accessible and transparent. This new referral process allows professionals, carers, agencies and service users themselves to refer into treatment. Once completed these forms need to be returned to the team base either through post or to the team by fax number. As mentioned earlier this form can be filled in by anyone involved in the service users care, including the service user. Once the forms have been received by the team then they will be triaged at the team meeting. This triage will generate an initial assessment appointment and appropriate treatment can be discussed with the service user.
Alternatively a referral can be made by telephoning the service and referring by giving details of the person to a member of staff at the team base. The referral will then follow the same process as outlined above.
The only changes to this process occur when a person is involved in either the Criminal Justice or Primary Care aspect of the scheme. These referral pathways are outlined in the areas of this leaflet that pertains to such areas of treatment provision.
