One to one support
- We will undertake an initial assessment of need which will help us determine the level of support appropriate for young people. This can mean advice, information, education, harm minimisation, initiating a CAF or signposting to other services for example. We will undertake a more comprehensive assessment if necessary
- Each young person will be assigned a key worker.
- They will agree a plan together which will be reviewed on a regular basis. The plan may include other services if the young person has additional needs and will take place via a child wellbeing meeting as appropriate
- The assessment and plan will involve parents/carers whenever possible and appropriate.
As no young person generally has substance use needs exclusive to other needs, we believe it is important to work with other professionals involved in their care.
Young People's consent is crucial to maximise their opportunity to benefit from our service. As such, we ask that young people's consent is gained prior to referral.
