Summary of the outputs

  • To show an increase in the numbers of patients moving from specialist prescribing into enhanced shared care with the intention to move from there, where possible, to leave the service, drug-free or to achieve settled and fulfilled lives on maintenance treatment in line with National Treatment Agency and DAT treatment plans.
  • To provide an increase in access to a range of evidence based structured psychosocial interventions and wraparound care delivered by members of the multidisciplinary team, and also through joint working with other specialist agencies such as Job Centre Plus, Foundation Housing and Citizens Advice Bureau.
  • To provide facilities for Narcotics Anonymous and Alcoholics Anonymous.
  • To provide facilities for parents and friends advice through GASPED.
  • To ensure that services are better publicised and perceptions of drug and alcohol treatment are improved through the provision of drug and alcohol awareness and other education services for patients, carers and members of the public.
  • To provide increased access to treatment outside traditional working hours through the development of supplementary prescribing which will be available at the adjacent Ash Grove Pharmacy.
  • To provide a needle exchange service through Ash Grove Pharmacy.
  • To provide a limited dual diagnosis service.
  • To involve service users in the development of the service.
  • To provide service user questionnaires and other Patient Reported Outcomes Measures / Methodologies to help identify needs and shortfalls of the service.
  • In addition to information collected routinely through the established National Drug Treatment Monitoring System (NDTMS) and the WISMS Commissioning for Outcomes program, the enhanced service will collect and report on the following:
    • Those metrics required by the Department of Health Integrated Care Pilot program
    • Those metrics required specifically by those patients who are referred through the Improving Access to Psychological Therapies (IAPT) program.
    • Assessment of Alcohol use and READ coding of low risk, higher risk and dependent drinkers through validated alcohol screening methodology - targets to be set to reduce numbers drinking at dependent and higher risk levels.
    • Smoking data - targets to identify quit rates.
    • Diet and Exercise data - targets to increase physical exercise and adopt healthier eating patterns. Also to explore the use of 'Exercise on Prescription' in local gymnasiums.
    • Primary care mental care - to identify levels of mental ill health using measurements such as PHQ9 and GAD7 and instigate treatment along NICE guidelines. To provide group work for management of anxiety and depression.
    • Management of long term conditions - targets to achieve best levels of symptom control and to reduce risks of relapse across the range of chronic diseases.
    • Dental care - to help improve access to local dentists.
