This pilot involves a partnership between the WISMS Shared Care team and Ash Grove Medical Centre in the first instance, to provide clinical and social integration services for substance misusers.

The Enhanced Shared Care Service will bring the following benefits to patients:

• Substance misuse and wraparound care to substance misusers closer to the patients' home.
• Specialist services delivered alongside and fully integrated with established mainstream primary care services.
• Access to a range of evidence based psychosocial interventions and social reintegration services such as housing support and employment advice in the surgery setting thereby increasing attendance, reducing travel time and improving the patient experience.
• Access to a one hundred hour pharmacy through Ash Grove pharmacy improving access to prescribing for service users who may find it difficult to attend during normal working hours.
