WISMS has been selected by the Department of Health to be one of the services to be in a trial to see how health and other services can work better together and improve services for you. The trial is called the Integrated Care Organisation (ICO) and we are very proud of being selected and we need you to help us to help you.
Part of the ICO is to share information about you with all the organisations that are involved and will include Turning Point, Housing, Doctors, Social Services, Police, Probation, Job Centre Plus, DWP and other support services. We need to be able to exchange your name, date of birth, full address, NHS number or National Insurance number and why you have been to those organisations. The main project is looking at how we measure benefits to your health and wellbeing as well as other issues that may matter to you and your families including debt, accommodation, personal development, employment etc.
We have developed a way of measuring the progress of our service users and we aim to make this information available to you and to the staff involved in working with you, so that we are all able to see how well we are doing and what, if anything, needs to improve.
Service Users will be asked to sign consent form to exchange information between the participating organisations so some service users and staff have had questions about this information exchange and these questions and answers can be found in this Word document.
