Following a significant service redesign 5 years ago, a unique collaboration between Dr Linda Harris, NHS Wakefield District and Sean McDaid one of the Nurse Consultants employed by South West Yorkshire Mental Health Trust has led to improved outcomes for service users with substance misuse problems in the Wakefield District.

Working closely with other partners they have become local, regional and national innovators within their respective areas of expertise and together have developed sophisticated care pathways for vulnerable groups who often historically fell between gaps in services.

South West Yorkshire Mental Health Trust, as it moves towards Foundation Trust status, is keen to develop more collaborative partnerships and would be keen to support this integrated care pilot programme bid.
