Outreach support

We have an assertive outreach worker who provides intensive community support to identified clients who do not effectively engage with health services. This allows us to provide flexible access to services in many different settings.

The assertive outreach worker aims to establish a trusting relationship with each client in a flexible, creative and needs focused way. This helps us to provide a health and social care package that fits each clients own specific needs.
