Tier 4 Assertive Outreach Nurse

Assertive Outreach is a way of working with identified clients who do not effectively engage with health services, providing intensive community support to clients in their own environment. This flexible approach allows clients to access services in many different settings.

The assertive outreach worker aims to establish a trusting relationship with each client in a flexible, creative and needs focused way, that enables the delivery of a health and social care package that fits each clients own specific needs.
