• To develop integrated nurse pathways
• Improve hepatitis screening and vaccinations
• Case manage clients with complex health needs and concentrate on those with long term conditions who continually fail to access mainstream healthcare
• To be pro-active in harm minimisation and to work with clients to actively encourage healthier lifestyles and engage with primary healthcare where appropriate
